Mental Health Care Under Medicare: Better Than Most People Assume

Outpatient therapy pays at the same eighty percent as a cardiology visit, and the old discriminatory limit is gone. The friction is finding a clinician who takes Medicare.

Mental health coverage under Medicare is broader than most people expect, and the old rule that paid only half for therapy was removed a decade ago. What remains difficult is not the benefit but the supply: clinicians who accept Medicare are unevenly distributed and often booked out.

Outpatient care pays like any other medical visit

Part B covers outpatient mental health services at eighty percent of the approved amount after the annual deductible, the same share as a cardiology appointment. That covers visits with psychiatrists, psychologists, clinical social workers, psychiatric nurse practitioners, and family therapists who take Medicare.

The remaining twenty percent is the part people underestimate, because therapy is recurrent. Weekly sessions for a year generate a meaningful balance, which a Medigap policy covers and Original Medicare alone does not. Medicare Advantage plans cover mental health as a Medicare benefit but usually inside a network, so the first question is whether the clinician is in it.

Intensive and inpatient treatment

Partial hospitalization, which is a structured day program rather than an overnight stay, is covered under Part B. Inpatient psychiatric care in a general hospital is covered under Part A with the usual deductible and coinsurance. Care in a freestanding psychiatric hospital carries a lifetime limit of one hundred ninety days, a restriction that exists nowhere else in Medicare.

Medication management is covered as a medical visit, while the prescriptions themselves fall to Part D. Since 2013, Part D plans may cover benzodiazepines and barbiturates for certain accepted indications, which closed a long-standing gap for people treated for anxiety and seizure disorders.

Depression screening and prevention

Annual depression screening in a primary care setting is covered at no cost. Intensive outpatient programs, family counseling when it is part of treatment, and telehealth visits for mental health all fall within the benefit, and telehealth eligibility for behavioral health has been broader than for other specialties in recent years.

Practically finding someone

Start with the Medicare Care Compare directory, then call and confirm the specific clinician is accepting new patients and takes assignment. Ask whether the practice bills as a clinical social worker or a psychiatrist, because the reimbursement differs and so does the pool. If nothing local is available, telehealth practices that contract with Medicare have widened the search considerably.

The short version

Therapy is covered at eighty percent with no visit limit, and inpatient psychiatric care carries a one hundred ninety day lifetime cap. Budget for the twenty percent, and expect the search for a clinician to take longer than the coverage question.
