Two Homes, One Medicare Card: Coverage for Snowbirds

Original Medicare plus a supplement travels the country without asking permission. A Medicare Advantage network does not, and that is where winter plans unravel.

Every winter, several hundred thousand people split the year between two states and assume their Medicare card works identically in both. Whether it does depends entirely on which of the two Medicare paths they chose, and the answer is not close.

The path that travels without asking

Original Medicare is a national benefit. Any physician or hospital in the country that accepts Medicare assignment will see you at the same cost share you pay at home, and a Medigap policy follows you because Medigap is standardized nationwide and pays according to the plan letter, not the state on your card.

That portability is the quiet advantage of the supplement model, and it matters most to people with two addresses, two sets of doctors, and a six-month absence from either one.

The path that stays home

A Medicare Advantage plan is built around a local network. An HMO generally covers routine care only in its service area, with emergencies covered anywhere. A PPO allows out-of-network care at a higher cost share, which softens the problem but rarely eliminates it, and the out-of-network annual limit is often separate and higher.

Some plans offer a visitor or travel benefit that extends routine coverage to another region temporarily. It is worth asking about specifically, in writing, and asking how many months it lasts, because the answer changes from plan to plan each year.

Prescriptions away from home

Part D networks are pharmacy networks, not geographic ones, so a national chain is usually in network in both states. Independent pharmacies are where surprises happen. Before leaving, confirm the pharmacy near the winter address is in network and whether the plan permits a ninety-day fill, which is the simplest way to avoid refill problems altogether.

Mail order is the other reliable answer, and it eliminates the geographic question entirely for maintenance medications. Check that delivery can be redirected mid-season, since a shipment sent to the wrong state is more than an inconvenience.

Visiting is not the same as moving

Spending four months in Arizona is a visit for Medicare purposes, and it does not create a special enrollment period. Moving permanently does, and the permanent move is what allows a plan change outside the annual window. The distinction is where you are registered to vote, where your driver's license is, and where you actually live, not how long the drive is.

There is also a Medigap wrinkle. Supplement premiums are set by state and by the rating method the carrier uses there, so a permanent move can mean a different rate, and in some states a new application with health questions. Never cancel a policy until the new one is issued.

The short version

Original Medicare with a supplement travels anywhere in the country. An Advantage plan works in its network unless it explicitly offers a travel benefit, so confirm that benefit in writing before you rely on it for a season.
