Medicare Advantage Plans by State

Medicare Advantage is priced by county and shaped by the state you live in, from the local hospital systems that sponsor plans to the Medigap rules that decide whether the choice can be reversed. These 51 guides cover what is actually sold, and what it costs, where you live.

An Advantage plan is a county product

Medicare Advantage is administered through private plans whose service areas are drawn county by county, and prices, networks and extra benefits are filed for each one. Two people forty minutes apart can be looking at completely different plan lists, with different hospitals and different drug formularies.

That is why a national comparison site can be misleading. The useful question is never "which plan is best" in the abstract, but which plans are sold where you live, and whether their networks include the doctors you already see.

What changes at the state line, and what does not

The federal rules do not vary: the 2026 in-network out-of-pocket limit is $9,250, the annual election period runs October 15 to December 7, and there is a January to March window for one change. Drug coverage inside a plan follows the same Part D rules, including the $2,100 cap on covered prescriptions.

What does change is the market itself. Some states are dominated by local nonprofit HMOs, two still sell Medicare Cost plans alongside Advantage plans, and a handful guarantee issue on Medigap all year, which makes the choice between the two routes reversible rather than permanent.

The decision that outlasts the plan

Choosing an Advantage plan is easy to do and, in most states, hard to undo. Once you leave Original Medicare, buying a Medigap policy later can require medical underwriting, and a diagnosis in the meantime can mean a decline or a permanent surcharge.

The state guides below say plainly how reversible that choice is where you live, which carriers actually sell there, and what a plan costs in practice rather than on the brochure.

Northeast

9 guides

Local nonprofit HMOs and hospital-sponsored plans set the terms here, and Medigap is guaranteed issue in several states, which makes the Advantage decision reversible in ways it is not further south.

Midwest

12 guides

Attained-age Medigap pricing and underwriting after the first six months make the Advantage choice harder to undo, while two states still sell Medicare Cost plans alongside standard Advantage plans.

South

17 guides

The heaviest Advantage enrollment in the country, with $0 premiums, deep dual-eligible offerings and hospital networks that change from one metro to the next.

West

13 guides

Plan availability tracks population: dense metro networks on the coast and in Arizona and Nevada, and counties across the mountain states where no Advantage plan is sold at all.
